While healthcare is a fundamental societal right, Malaysia's public healthcare system is currently grappling with a fiscal sustainability crisis, characterized by escalating government expenditure and critical personnel shortages. Despite the potential of Islamic Social Finance to alleviate these systemic pressures, the deployment of health waqf remains fragmented and largely confined to small-scale initiatives. A critical structural gap exists, there is currently no integrated operational framework that synergizes the regulatory authority of the State Islamic Religious Councils (MAIN), the capital capacity of the corporate sector, and the advanced infrastructure of private hospitals. Without a specialized model, private healthcare remains inaccessible to the underserved, and waqf capital remains underutilized. Consequently, this study addresses this institutional disconnect by developing a practical waqf-based private hospital framework, providing a sustainable policy intervention to bridge the disparity between private-tier medical excellence and public-interest accessibility.
